Member Enrollment Checklist
Members will not be enrolled until all of the required documents are received. Documents must be received within 20 days of the member’s start date. If this date cannot be met through mail, you can scan and email copies and then mail hard copies later. Member’s start date will be adjusted to comply with the CNCS rule of enrolling members within 30 days of their start date if documents are received late, meaning days served on days prior to the official start date cannot count.
If any changes are made to these forms by supervisor or member during completion (names, dates or signatures are crossed out and changed) please initial and date the changes.

Required documents/actions prior to enrollment
____ Once recruited the Member's name and email address are sent to IACC in order to expedite the enrollment process
____ Enrollment Packet Sent to IACC (all originally signed copies must in possession of IACC):
_____ Sex Offender Check (Tutorial Available), must be a direct printout from the internet with date/time stamp. If there is a match, document how we know it does not match the member (name, picture, birth date, etc) on the print-out.
_____ FBI Background Check (Tutorial Available): if the member will have recurring access to vulnerable populations, an FBI background check is required. Site supervisors must fingerprint members (through local law enforcement or campus security) and send in check request information PRIOR TO the member's start date.
_____ Member's proof of identity and age: Typically this is a scanned copy of the member's driver's license or other government-issue ID
_____ Orientation Agenda with date of orientation and member signature
_____ Member completes the Pre‐Service Survey
_____ Member & Supervisor are both registered for time-keeping system (your site is your campus, you will not be able to see time sheets immediately, see attached instructions).
- AmeriCorps Application Form (signed and dated by member, date must be on our prior to member start date)
- Member Service Plan/Position Description, must include specific agencies and plans and indicate whether there will be recurring access to vulnerable populations
- Member Service Agreement (signed/dated before or on start date, end date should be the ACTUAL end date of service as indicated on the members timesheets)
